Medical: Blue Cross of California EPO
The Blue Cross EPO covers most services at 100% after a co-payment of $15. Contracted providers must be used to receive benefits. Employee cost: $31.17 per pay period for employee-only, $65.94 per pay period for employee plus one and $98.98 per pay period for employee plus family

Medical: Blue Cross of California PPO
The Blue Cross PPO pays a percentage of covered eligible expenses after an annual deductible is satisfied. However, only a $15 co-payment is required for office visits and no deductible applies. Under the PPO, employees are allowed to choose any licensed doctor or health care facility to provide health care. The PPO has a network of providers and facilities that are considered "preferred" providers. When these PPO providers are used, the plans pay a greater percentage of the cost of medical care, generally 90% versus 70% out of the network. Employee cost: $34.43 per pay period for employee-only, $72.81 per pay period for employee plus one and $109.26 per pay period for employee plus family.

Medical: Kaiser HMO
Headway’s Kaiser plan charges $15 per visit and pays 100% of most medical expenses when provided by Kaiser physicians and facilities. Employee cost: $35.67 per pay period for employee-only, $75.41 per pay period for employee plus one and $113.23 per pay period for employee plus family.

Medical Prescription: Kaiser
The Prescription Drug Plan Copay (for up to a 100 day supply) - Generic $10.00 and Brand $20.00

Medical Prescription: Blue Cross
The Prescription Drug Plan Copay has a three-tier co-payment structure:

  • At the Pharmacy (for up to a 30 day supply)
    • Generic - $10; Brand Named Formulary - $20
    • Brand Named Non-Formulary - $40.00
  • Mail Order (for up to a 90 day supply)
    • Generic - $20; Brand Named Formulary - $40.00
    • Brand Named Non-Formulary - $80.00
  • Self-administered injectible drugs (except insulin)
    • Employee pays 20% of the approved expense, up to a maximum out-of-pocket cost of $100 per prescription.

    Chiropractic Care and Mental Health Care

    All Headway medical plans provide for Chiropractic care and Mental Health Care.

    Dental: Blue Cross PPO

    The PPO Dental Plan covers a wide range of dental services, including dependent and adult orthodontia. The annual deductible under this plan is $50 Individual and $150 Family. The plan covers preventative care at 100%, basic care 80% and major care at 50%. There is an annual maximum benefit of $1,500.

    Vision Plan

    Headway offers a vision care program to employees and their dependents through EyeMed. An employee may receive benefits every 12 months, except for frames, which are every 24 months.

    401(k) Plan

    Headway offers a 401(k) Savings & Retirement Plan that allows an employee to save money and taxes at the same time by investing in a variety of funds. The plan is provided through Fidelity Investments and offers 26 investments to choose from. Employees can contribute 1% to 60% of their pay, up to a maximum of $15,500 in 2008 (subject to adjustments annually). Headway will match 100% of an employee's first 4% of contributions. The 401(k) Plan also allows an employee to borrow up to 50% of his or her account balance. Rollovers are accepted.

    Holidays

    Headway provides 11 paid holidays per calendar year.

    Paid Vacation

    Full-time employees earn 96 hours of paid vacation in their first year of employment. Paid vacation increases by 8 hours for each year of employment up to the employee’s eighth anniversary where they will earn 160 hours. Employees are allowed to "cash-in" one week of vacation each year in conjunction with taking one week off. The maximum vacation balance is 200 hours.

    Sick Leave

    Full-time employees earn 80 hours of sick leave each year. At the end of each year employees are able to convert any unused sick leave they earned during the year to vacation time at 50% rate.

    Life Insurance and AD&D

    Headway provides Life Insurance and AD&D Insurance at no cost to its employees. Coverage under each plan is 2 times your annual salary up to a maximum of $500,000.

    Short-term Disability

    Headway provides Short-term Disability Insurance at no cost to its employees. Coverage is 60% of earnings, up to a maximum of $3100 per week. Benefits begin after the 7-day elimination period.

    Long-term Disability

    Headway provides Long-term Disability Insurance at no cost to its employees. Coverage is 66 2/3 of earnings, up to a maximum of $13,500 per month. Benefits begin after 180 days of disability.

    Flexible Benefits

    Headway offers a Flexible Benefit Account for health only. Employee may contribute up to $4,000 pre-tax to a Health Care Account to pay for unreimbursed health care expenses for the employee and for his or her legal dependents. Eligible expenses include deductibles, co-payments, uncovered major dental work, orthodontia and approved over the counter drugs.

    Educational Assistance Program

    Headway's Educational Assistance program encourages personal development through formal education. Depending on grade received, Headway will reimburse employees up to 100% of their education expenses, up to $4,500 per year under this program.

    Long Term Care

    Non-ERISA Plan: Provided through Prudential company. All employees are guaranteed coverage without having to provide their medical history.
